Patient Policy

In order to provide all our patients with the best possible care, we request:
-
A consenting adult must be physically present in the reception
area while children 15 and under are being treated.

Adults accompanying patients are not allowed back to the operatory,
unless the doctor finds it necessary.

Three unexcused absences for dental appointments can be grounds
for termination of treatment.

Due to the size of our reception area, we only have room for the
patient(s) and the responsible party.

Patients will be cooperative and have good, respectful manners to
the doctor and the dental staff at all times.

The dental office staff will not provide babysitting services while
patients are being treated; child care arrangements need to be made elsewhere.

Patients will arrive on time for their dental appointments ... otherwise,
if late more than three (3) times, this can be grounds for termination of treatment.

No smoking in the dental office.

Payment is due at time of service.

If you are unable to keep your appointment, you must give at least
one (1) business day's notice or you will be charged a fee.
-
You must present (for copying) a valid Photo I.D. as per the
Federal Trade Commission rules.
